iStock_000000319521XSmallThis series of articles for families will focus on questions we get asked a lot: “How can I make my (husband, daughter, boyfriend, sister, etc.) stop drinking/using drugs?” “How can we make our (mother, cousin, girlfriend, brother, etc.) get help for their problem with (booze, cocaine, painkillers, etc.)?” We understand the problem: The drugs or alcohol have turned someone you love into a monster. How can we turn them back?

Part 1: The Lonely Fight

Gemma’s alone, smoking and staring at the wall after yet another humiliating disaster. This time Stan promised he’d remember. It was his own son’s birthday cake, after all. Long after the birthday party attendees had been sent home with excuses, he turned up empty-handed. And angry. It was Gemma’s fault again (as usual)- she was supposed to call and remind him. Except that she wasn’t. He promised. Why did I believe him? Gemma thinks. He’s broken so many promises…

Matt is watching the Late Show again, or pretending to. He’s actually waiting, hoping that his 18-year-old son will come home in one piece. And hoping that the inevitable row won’t escalate to physical violence. He’s given up on hoping the kid won’t be stoned or flying high on whatever substance crossed his path tonight. Seems like getting turned down for that scholarship turned him into another person, he thinks. Or was it the drugs?

Maggie cuddles up to her iPad, reading yet another article on the symptoms of alcoholism. Her husband hasn’t lost his job— in fact, he just got promoted a year ago. But in a few minutes, she’ll have to go down and check to make sure he hasn’t fallen asleep in his “man cave” with a cigarette burning, and an empty bottle of Scotch nearby. One of these days, she worries, he’ll burn himself— or the house. But he can’t be an alcoholic. He’s got a good job.

Carlos keeps promising himself he’ll get a divorce from his wife once the kids are grown and gone. But the youngest just got arrested for shoplifting and tested positive for marijuana. And although he’s had to clean up after two or three “binges” a year— all the way to committing her to a psych unit at the local hospital— he still feels responsible for his wife. If I leave her, he thinks, she might even kill herself.

Gemma and Maggie, Carlos and Matt, wouldn’t think they had much in common. Each of them feels very much alone. Yet they are all living with, and suffering from, the same disease— one that, if left untreated, will probably prove fatal to their loved ones. And maybe to them as well.

But this disease can be treated. And people can recover. They may never go back to being the person they “used to be” before the alcohol or drugs consumed them, but they can change again— for the better. Recovery requires the alcoholic or drug addict to do three things:

  • Learn some new information and change a few crucial things about the way he or she lives;
  • Follow certain key directions, whether or not they seem convenient; and
  • Once the disease is in remission, make sure she or he doesn’t stray too far from the recovery regimen.

These things sound simple, but they’re not at all easy. In return, however, the victim of addiction gets something unusual in the treatment of fatal illnesses: a virtual guarantee of survival and the opportunity to live a normal life.

People like Maggie and Matt, Carlos and Gemma, spend months or years trying to determine whether or not their loved one is an alcoholic or drug addict. Then more time trying to decide what to do about it. And in trying to “manage” the problem, they often end up doing just about everything that can make it worse, not better. Such as:

  1. Giving the addict/alcoholic responsibilities and then getting angry when they don’t fulfill them. Like Gemma, who knows that Stan always stops for a drink or four on his way home. But she gives him an errand crucial to the success of her son’s birthday party, because she felt he should bear some of the responsibility. Understandable, but unrealistic.
  2. Being fooled by apparent lulls in the drinking or drug use. Carlos often tells himself his wife is “getting better,” when she goes for months without a major binge. She’ll be less irritable and difficult, even have periods of functioning well. But the reality is, these periods always end, and the problem escalates again. If she were going to spontaneously “get better” it would have happened already.
  3. Making excuses for the addict or alcoholic. Matt believes that his son is a good, bright kid whose increasingly erratic behavior and drug use were “brought on” by the disappointment of losing a scholarship. He explains away the problems: “Kids have to sow wild oats.” “The cop had it in for him.” “The other boys involved were the ringleaders.”
  4. Getting in arguments or trying to “discuss” the problem when the alcoholic has been drinking. The picture of futility: Expressing anger to someone whose brain is distorted by powerful chemicals. No matter how valid the points Maggie makes to her husband may be, he won’t hear them. And he certainly won’t remember them the next day. But he might take a swing at her one of these days.
  5. Getting advice from friends who don’t understand addiction either. Friends at work, other family members might well advise Gemma to leave. Or tell Matt to kick the kid out. They won’t take the advice, and the advisors will get increasingly frustrated—and stop listening, stop offering suggestions. And increase the sense of loneliness and isolation.
  6. Questioning their own sanity. Alcoholics and addicts are very good at turning deflecting criticism by criticizing the criticizer. Am I overreacting? Maggie asks herself sometimes. Carlos hears so much about his own inadequacy as a husband, father, etc. he may end up needing therapy even if his wife manages to recover.
  7. Trying to protect the alcoholic or addict from danger. No matter how many times they rescue their loved one from consequences or head off the problems of addiction, they’ll just keep happening. Until something is done about the disease that puts them in danger, the alcoholic will find the car keys, stumble on the stairs, the addict will disregard safety… It’s futile, and foolish to keep trying.
  8. Trying to handle the problems without help. Isolating themselves from real guidance, people like Gemma find it easy to escalate feelings of helplessness and inertia. Or they seek advice from friends which may be well-intentioned, but pretty much useless. Like asking your brother-in-law how to treat diabetes (unless he’s an endocrinologist, of course.)

Here’s the good news: Although there’s little that we can do to “make” someone we love stop drinking, or using drugs, there’s quite a lot we can do to help them decide to change.

Great article Scott! There is help for the codependent who struggles with doing the same things over and over and expecting different results. I agree with Lisa Neumann’s comment that self care is essential. I would also like to add that if someone is not used to taking care of themselves, it is foreign and support and guidance is key. Alanon and Coda are great resources for this. Also, working with a licensed professional who understands addiction is crucial when an addict’s loved one is suffering from the disease to the extent that it severely impacts their daily functioning.

So many great points in this article. Seems I often forget what active addiction is like. Again, the reason to stay close to the “recovery regimen.” I never want to go back to that place as the addict or the co-dependent. continued work on self and it’s actually rather rewarding. Your site is so wonderful. Another excellent post to share.
